The difference in boat speed can be considerable.  If the helmsman is clutching his chest in great pain then he has no free hands to fine-tune the mainsheet, kicker, tiller etc. then boat speed will drop off and the boat will be unable to point.  This can be a real race-looser, which is why people wear pacemakers and even go to the lengths of having them surgically implanted so no-one can see them cheating!

You sail along for a bit close hauled on each tack and it gets the idea of which way you should be pointing. If you get headed by more than a certain amount it tells you to tack.
